After startup it immediately dies

I have had this issue through ownership of the car and I can't seem to find a pattern to it. The car turns over no problem then dies, if I apply a little gas the car stays on. It only happened 25% of the time. I heard that it might be the EGR valve. Has anyone else fixed or addressed this issue?

How old is the vehicle? That's really odd but I would imagine the dealership would start of a reflashing of the ecm.
